Mail Option Registration Information and Fees

HOW TO REGISTER AND COMPLETE THIS COURSE

  • REGISTER IN ADVANCE on-line at www.universitylearning.com, call Toll-Free at 1-800-940-5860, by Mail or FAX at 703-689-4452 (using the form on the back page.)
  • Please indicate: (a) where you will be taking the program; (b) the dates you are departing for that location and are returning home; and (c) the dates you will be completing the program. Please let us know the location where you would like the discs shipped.
  • About 4-7 days prior to your departure, you will receive the complete course, including disks, the compre-hensive handout prepared by Dr. Weart and a written program evaluation form. Pharmacists will also re-ceive a quiz and answer sheet.
  • Shipments are normally sent by FEDEX Ground. Rush fee is $20.00. Shipments to Canada, Alaska and Hawaii are an additional $20.00. Discs will be shipped within the USA and Canada only. Shipments to Canada may require 7-10 days to arrive.
  • Take the course with you to the site of your choice, or do it at home.
  • Return the discs and associated materials by the return date in your instruction letter.
  • Upon receipt of the discs program materials, ULS will mail your statement of credit to you.
  • ULS does NOT pay for return shipping.

Obtaining CE Credit

  • After completing the course you must complete a multiple choice quiz in addition to the written program evaluation. Send your evaluation form and quiz answer sheet to us for processing .
  • Upon obtaining a grade of 70% or better, you will receive certification of CE credit from University Learning Systems (which is recognized by every state board of pharmacy).
  • If a passing grade is not obtained, you will be allowed to retake the quiz. A passing grade of 70% or higher and a completed written program evaluation is required in order to earn CE credit.
  • Your grade will be mailed to you with your CE Certificate.

Refund Policy for Mail Option

  • Cancellation prior to shipping the course materials are subject to a processing fee of $50 per person.
  • A refund of your registration fee less the processing fee will be issued via credit card or check (depending on how payment was originally made). Or you may apply your registration fee, less $25 to another course (live or DVD/CD) any time within 12 months. 
  • Once application of the registration fee to another program is chosen, there are no further refunds or application to another program should you cancel again.
  • Cancellation once the course materials are shipped is subject to a fee of $75 per person.
  • A refund of your registration fee, less the processing fee will be issued provided the complete course materials are returned to us within one week after you receive them. Otherwise, no refunds are issued.
