Earning Credit at a Hospital Site:

  • Select the HOSPITAL LOCATION
  • Select the consecutive THURSDAYand FRIDAY DATES (for example Thrsday, January 7 and Friday January 8)
  • Register in advance by web, mail, phone or fax
  • Walk-in registration is not permitted.
  • Upon receipt of your seminar registration, ULS will send you confirmation and attendance instructions, including directions to the hospital location and your hospital contact person.
  • At the hospital, you will be provided with the discs and course materials, including syllabus, test, answer sheet and program evaluation.
  • Upon hospital verification of attendance, a passing grade of 70% on the test if required, and receipt of your evaluation form, your statement of credit will be sent to you by mail.
  • For AAFP Prescribed Credit, AMA PRA Category 1™ Credit, ACPE, or AAPA Credit, you must receive a score of 70% or greater on the multiple choice test and and complete the written course evaluation.
  • For AMA Category 2 Credit, and Nursing Credit, you complete the written course evaluation. No test is required.

Hospital Option Refund Policy

  • Notice of cancellation must be in writing via mail, fax or e-mail.
  • Cancellation up to 5 days prior to the program date is subject to a processing fee of $50 ($25 for accompanying spouse), or you may apply your registration fee, less $25 to another program within 12 months.
  • Only one rescheduling permitted.
  • No refund given for cancellation less than 5 days prior to the seminar date, although you may apply seminar fee, less $50 processing fee, one time to another DVD course, or live seminar within 12 months.
  • No refunds or credit for nonappearance. No refunds for partial attendance.
  • No refunds due to airline cancellation or delays.
  • Once application of the registration fee to another program is chosen, there are no further refunds or application to another program should you cancel again.
  • Refunds may take 2-4 weeks to process after notification of cancellation.
