Registration Instructions

Please indicate:

  • Where course should be shipped (home, office, course location)
  • Course location (Where you intend to complete the course.)
  • Departure date to course location
  • Return date
  • Course date(s)
  • About 4-7 days prior to your departure, you will receive the complete course, including discs, the comprehensive syllabus prepared by Dr. Weart, test, answer sheet, and a written program evaluation form.
  • Shipments are normally sent by FEDEX Ground and require 7 business days notice. Rush fee is $25. Shipments to Canada, Alaska and Hawaii may require an additional surcharge of $25 depending on shipping method. Discs will be shipped within the USA and Canada only. Shipments to Canada, Alaska and Hawaii may require 7-14 business days to arrive.
  • Take the course with you to the site of your choice.
  • Complete the course, test and evaluation.
  • Return the discs, answer sheet ( if required) and evaluation by the return date in your instruction letter.
  • Upon receipt of the discs, test and evaluation, ULS will mail your statement of credit and test results.
  • ULS does NOT pay for return shipping.

Obtaining CE Credit

  • For AAFP Prescribed Credit, AMA PRA Category 1™ Credit, ACPE, or AAPA Credit, you must receive a score of 70% or greater on the multiple choice test and and complete the written course evaluation.
  • For AMA Category 2 Credit, and Nursing Credit, you complete the written course evaluation. No test is required.
  • Please send your evaluation form and test answer sheet with the discs to ULS . Upon receipt, ULS will send your statement of credit and test results.

Refund Policy for Mail Option

  • Cancellation up to 7 days prior to the program date or shipping date is subject to a processing fee of $50 per person, or you may apply your registration fee, less $25 per person to another program within 12 months.
  • No refund given for cancellation less than 7 days prior to the course or shipping date, although you may apply course fee, less $50 processing fee to another program (live, hospital, or mail) within 12 months.
