Upon completion of this program, participants should be able to:

  1. Identify a working definition of medication errors
  2. Identify the different causes of medication errors in the Community pharmacy workplace
  3. Develop strategies to improve systems as a key way to reduce medication errors
  4. Evaluate errors and identify if they are caused by improper prescribing, dispensing or administration
  5. Evaluate one state’s attempt to analyze errors and a non-punitive reporting system
  6. Recognize the importance of patient counseling in the prevention of medication errors in the pharmacy.
  7. Evaluate procedures regarding the handling of medication errors in the pharmacy.

  8. Discuss Root Cause Analysis (RCA)and Continuous Quality Improvement (CQI).
